Your first step is to obtain Physician eCertification After your visit you will be notified in the Patient Portal when your ecertification has been issued. Applications must be submitted to the state within 30 days of doctor’s signature. if you have not visited with the physician please call us 314-596-9955
Next Register or Login to Missouri Application Portal (MMMP) if this is your first year applying for your medical card you will need to register. If you are renewing your card or have previously created an account you will login with your email address and password: MMMP
Click ‘Create New Application’ For the Application Type click the ‘I am a’ drop down box, and select ‘Patient’
If you are a new patient, select ‘New Patient Registration’. If you are renewing an application, select ‘Patient Renewal’. Click ‘Create Application’.
Fill in the details on the General Information tab. Click ‘SAVE & NEXT
Continue entering the details on each page clicking ‘SAVE & NEXT’
When completing the PHYSICIAN / CONDITION INFORMATION tab select the green box VIEW AVAILABLE CERTIFICATIONS A pop-up will appear listing available certifications. Choose the electronic form and click ‘Update’. The information submitted by the physician will auto-populate into the application. Be sure to review the information provided in the electronic certification for accuracy prior to submission of your application
If a tan box appears indicating that ‘No Physician Certification found” this means either the physician has not submitted the form, or the information does not match your account. To correct the errors, please contact the physician first to determine if the error is within the form, or within your account, then contact the department.
Continue through the application clicking ‘SAVE & NEXT’. After you have completely reviewed your patient application on the review tab and there are NO Red Xs, you can proceed to processing your payment. Click ‘Pay & Submit’.

After your application is submitted you will receive a confirmation email from MMMP. They will email again once your application is complete. This can take up to 30 days but is usually around 2 weeks.
Once approved you will received a digital medical cannabis identification card in your MMMP account.

Printed Plastic Cards
If you have paid for our office to mail you a plastic card please message us in the patient portal when you receive notification from MMMP that your card is ready. You can either provide us with your Digital Card your MMMP account login information.
