Physician consultations are $150 for your 3 year license
HOW DO I SCHEDULE AN APPOINTMENT TO TALK WITH THE DOCTOR?
No need! First complete the Medical Intake Form, then when you submit all of the required information you will be forwarded to the online waiting room to start your visit. The doctor is available Monday – Saturday 10am-8pm, Sundays 12pm-6pm, excluding Holidays. If you complete the Medical Intake Form but you are not able to visit at that time you will be sent a link to the online waiting room in the Patient Message Portal.
WHAT DO I NEED FOR MY VISIT?
Prior to talking with the doctor you will complete a Medical Intake Form online. In this form you will complete your health history, provide a photo of yourself, a photo of your State ID, and enter your payment information. You should bring any medical records that you have that relates to your qualifying medical condition. The form should be completed on a device equipped with a camera as you will be automatically forwarded to the online patient waiting room to start your video telehealth visit with the doctor.
WHAT IS A TELEHEALTH VISIT?
Telehealth visits allows patients to meet with the physician on a video call through a computer or mobile device. Telehealth is a convenient option allowing patients to meet with the doctor from the comfort of their home.
After completing the intake form you will automatically be taken to the online waiting room to start your visit.
WHAT DO I DO AFTER I TALK TO THE DOCTOR?
After your visit please check your email for an invitation to register for our Patient Message Portal. This is our primary form of communication.
We will notify you in the Patient Message Portal when you can submit your application to the state for your license. Applications MUST BE SUBMITTED WITHIN 30 DAYS FROM THE DATE OF THE DOCTOR SIGNATURE on your certification.
All patients will need to register and apply through the Missouri Medical Marijuana Portal (MMMP) https://mo-public.mycomplia.com/
HOW MUCH DOES MISSOURI CHARGE FOR MY CARD?
Once you have completed your online application, linked the electronic physician certification and uploaded all other required documentation, the online application fee must be paid to Missouri’s DHSS.
The state application fees are separate from Greencert MD’s visit fees unless you choose the optional services for Application Assistance.
Standard state application is $25*
Patient Cultivation Fee (optional) is an additional $50*
Caregiver application fee (optional) is $25*
*All fees listed above will also be subject to additional processing fees. The state does adjust these fees annually.
All application fees MUST be paid with a Visa, MasterCard, or Discover credit or debit card. If you do not have a credit or debit card, you must find a retail store that offers prepaid Visa or MasterCard gift cards and purchase one there.
Missouri Marijuana Card
The state of Missouri does NOT mail out physical cards to approved patients. Once approved by the state, the patient receives an email notification with instructions to log into the online portal to access their digital medical marijuana card. This card can be printed, or saved to their electronic device. Greencert MD does offer professionally printed cards. Please visit HERE for more information.
DOES MY MEDICAL MARIJUANA CARD EXPIRE?
Yes. Your card is valid for 3 years. You should start the renewal process 30-60 days prior to your expiration date. Our office will email you a reminder.
CAN YOU HELP ME APPLY WITH MISSOURI FOR MY CARD?
Yes! We do offer a full Application Assistance service. This service includes the fee Missouri charges for your application. Our staff will register you on MMMP and complete all information on your application. You will be required to verify your email address in an email sent to you from MMMP. If you have already created an account on MMMP we will need your password for the account to complete your application. This is a service done on behalf of the patient so we will need to log in as the patient.
I DON’T HAVE ACCESS TO THE EMAIL I USED FOR MMMP CAN YOU HELP?
The state requires that the patient contact them to change the email on their account. You can call them at 866-219-0165 or email them medicalmarijuanainfo@health.mo.gov
You will need to provide them with your Full Name, date of birth, last 4 of your SSN and your new email address.
