Evaluations are $99. If we are unable to certify you for a qualifying medical condition you will be refunded.

Once you have completed your online application, linked the electronic physician certification and uploaded all other required documentation, the online application fee must be paid to Missouri’s DHSS.

The state application fees are separate from Greencert MD’s fees unless you have paid for the application service.

Standard state application fees are $25
Patient Cultivation Fee (optional) is an additional $100
Caregiver application fee (optional) is an additional $25

All fees listed above will also be subject to additional processing fees. The state does adjust these fees annually.

Application fees MUST be paid with a Visa, MasterCard, or Discover credit or debit card.

Missouri Marijuana Card
The state of Missouri does NOT mail out physical cards to approved patients. Once approved by the state, the patient receives an email notification with instructions to log into the online portal to access their digital medical marijuana card. This card can be printed, or saved to their electronic device. Our office does offer printed plastic cards to be mailed to you.

No need! First complete the Medical Form, then when you submit all of the required information you will be forwarded to the online waiting room to start your visit. The doctor is available Monday – Saturday 10am-8pm, Sundays 12pm-6pm, excluding Holidays. If you complete the Medical Form but you are not able to visit at that time you will be sent a link to the online waiting room in the Patient Portal.

Prior to talking with the doctor you will complete a Medical Form online. This form collects:
health history
photo of you
photo of your State ID
payment information (debit or credit card required)
any any medical records that relate to your qualifying medical condition
The form should be completed on a device equipped with a camera as you will be automatically forwarded to the online patient waiting room to start your video visit with the doctor.

After your visit please check your email for an invitation to register for our Patient Message Portal. This is our primary form of communication.

We will notify you in the Patient Message Portal when you can submit your application to the state for your card. You are not legal until you have received your card from the state. Applications  MUST BE SUBMITTED WITHIN 30 DAYS FROM THE DATE OF THE DOCTOR SIGNATURE on your certification.

All patients will need to register and apply through the Missouri Medical Marijuana Portal (MMMP)

Yes. Your card is valid for one year. Prior to the expiration of your card you should contact us to request a Returning Patient intake form and have a follow up visit. If we have your current email address we will email you a reminder.

Yes! We do offer a full application service. This service includes the Missouri application fee. Our staff will register you on MMMP and complete all information on your application. You will be required to verify your email address in an email sent to you from MMMP. If you have already created an account on MMMP we will need your password for the account to complete your application. This is a service done on behalf of the patient so we will need to log in as the patient.
